The HEI application form can be downloaded below in either Microsoft Word or PDF format.
HEIs will be advised by the Education Team whether the visit will be Type 1 or Type 2 and should therefore complete the relevant application form.
As far as possible the HEI application form has been designed to allow institutions to present information in a similar format to that required by the Quality Assurance Agency (QAA) as Programme Specifications documents.
This is particularly the case with Section B of the application form relating to Programme Issues. BCS does, however, require specific additional information not requested by the QAA and you must ensure that each of the questions in the application form is addressed.

The following forms must be used to provide a table for each programme mapping the core modules to the requirements for the accreditation sought (required for both Type 1 and Type 2 applications in section B2.4 of the applications forms.
The following form should be used to provide a table indicating which modules are mandatory and which are optional (including credit value), thereby showing commonality across all programmes submitted for accreditation. This is required for both Type 1 and Type 2 applications in section B.2.3 of the application forms.
If documents are not received within eight weeks of a visit, or do not adhere to these guidelines, BCS reserves the right to postpone the visit.
